Patient Portal Help

Welcome!

The patient portal is a website, much like the websites you may use to bank, invest, or check school records online. In our case, it lets you send and receive communication with your provider's office when it's convenient for you!  It is tied to our electronic health record so your doctor can easily send information from your hospital or clinic chart to your portal account.

How to accept your invitation to the patient portal.

  

What you can do with your patient portal:

  • Request an appointment time and look up your schedule appointments online
  • View your bill and pay it online
  • Receive your test results faster than through the mail
  • Ask your provider a question or contact the billing department when it's convenient for you

You will only receive an e-mail to alert you that information has been sent to your portal account. You will not receive junk emails and your e-mail will not be shared.

It is FREE, quick and easy to use, and your information is secure.

Proxy access for family members or dependent adults is also available.

Please complete and return this form designating a personal representative to your provider's office to request proxy portal access for an adult family member.

  

Proxy Form FAQ

Should I fill out the proxy form to sign up for Patient Portal?

No, this form should ONLY be filled out to provide account access to an adult representative over 18.  To sign up for the Patient Portal, you must fill out the Patient Portal Access Request Form.

Can I sign up with the same email address as my spouse who has already registered for Patient Portal?

Yes, however the owner of the other account must fill out the proxy form to share the same email with the new account.  The new account must complete the access request form to sign up.

Can I fill out the proxy form to give access to a family member that is not a patient of Hansen Family Hospital and Clinics?

No, proxy account access can only be granted to our hospital and clinic patients.

Does my minor child need to fill out the proxy form to give me access to their account?

No.  Legal guardians of children ages 11 and under will have automatic access to the minor child's information and do not need to fill out the proxy form.  Certain personal health information pertaining to mental health, reproductive health, and substance abuse is legally protected for minors ages 12-17; therefore legal guardians will not be able to obtain proxy access for minors ages 12 and up.
